Findings: The Alleged Victim (AV) relies on the facility for care and to provide a safe environment. According to an investigation, Witness 1 (W1) and AV engaged in three resident to resident altercations from June 7, 2025, through June 11, 2025, and W1 was not administered their cognitive impairment behavioral medication from June 6, 2025, through June 8, 2025. The facility failed to implement any non-medication interventions to mitigate AV and W1 from having continued resident to resident altercations, which is a violation of resident rights and is neglect, which constitutes abuse.
