Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is known to have an allergy and allergen information was noted on the dietary spec sheet and on a large whiteboard in the kitchen. According to an investigation, on or about June 10, 2025, AV ordered and was served an item at dinner that contained AV’s allergen, resulting in unreasonable discomfort and emotional harm. The facility failed to follow the care plan, which is considered neglect of care and constitutes abuse.
