Findings: Alleged Victim (AV) had food allergies to multiple foods including onions. On or about June 14, 2025, AV was served a breakfast that included onions. Following AV’s exposure to onions, AV was observed repeatedly falling in and out of consciousness. Alleged Perpetrator 2 (AP2) was aware of AV’s allergies. AP2 provided and server the plate containing an allergen that was served to AV. AP2 failed to follow the care plan, which is neglect of care and constitutes abuse. The facility failed to assure AV’s service plan was being followed, which is a violation of Oregon Administrative Rules.
