Findings: The Alleged Victim (AV) has cognitive deficits and is dependent on facility staff to meet his/her care needs. According to an investigation, on or about June 17, 2025, AV was visited by a previously terminated employee, Witness 14 (W14), and asked to sign a document, which caused AV to feel deceived, worried and fearful. W14 made multiple visits to AP1 since being terminated in January 2025. The facility had not taken any action to restrict W14's visits, which was against facility policy. The facility failed to protect AV from emotional abuse, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
