Findings: The Alleged Victim (AV) relies on the facility to provide for their care needs and a safe environment. Witness 1 (W1) and AV have had previous resident to resident altercations. W1 and AV were both in the dining area when AV touched something on the table near W1 and W1 struck AV. The service plan for W1 was not being followed at the time, as W1 should have had a one-on-one caregiver present. The facility failed to provide AV with care and a safe environment, which is a violation of resident rights and is neglect, which constitutes abuse.
Sanction: RCFCP25-01037 $375.00 fine assessed
