Findings: Alleged Victim (AV) resides at Respondent’s facility. Facility staff knew AV had history of falls. Staff are to ensure pathways are clear and clutter free. On June 12, 2025, AV was found on the ground injured in AV's bathroom. AV reported hitting h/h head and having pain everywhere. Although AV's service plan indicated staff are to ensure pathways are clear and clutter free, staff failed to provide a safe environment and follow the service plan. There is a large armoire across the doorway area against the wall in AV's room. The bathroom door swings back into the room leaving no room for the AV to get buy using a walker. Respondent failed to ensure staff provided a safe environment and follow the service plan. This constitutes abuse by neglect.
