Findings: Witness 1 (W1) had a known history of aggressive behaviors prior to being admitted to the facility. On or about June 11, 2025, facility staff discovered W1 attempting to have nonconsensual intercourse with Alleged Victim (AV) in W1’s room. W1 was found completely nude, erect, and holding AV from behind by AV’s waist. W1 would not permit facility staff to remove AV from W1’s room and stated that W1 was going to kill facility staff when they would not leave W1’s room. As a result of the incident, AV put AV’s self in a fetal position and started crying after the facility staff were able to interrupt W1. The facility failed to protect AV from inappropriate sexual contact and emotional abuse, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-01259 $500.00 fine assessed
