Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, AV has a history of repeated falls and AV's care plan indicates AV is independent with ambulation and transfers and does not mention falls. On or about June 22, 2025, AV had an unwitnessed fall resulting in a hospital visit where AV was diagnosed with a head injury. The facility failed to implement effective interventions to mitigate AV’s increasing fall risk and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
