Findings: The Alleged Victim (AV) relies on the secured memory care facility for his or her safety. On or about June 22, 2025, AV eloped from AP1 through an unsecured courtyard gate. AV was able to open the gate, walk across a busy street to a bus stop, and was out of the locked memory care facility for approximately 40-60 minutes. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01274 $375.00 fine assessed
