Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is known to prefer to be in their apartment alone. Witness 1 (W1) had a known history of sexually inappropriate behaviors. According to an investigation, on or about June 27, 2025, W1 entered AV's room and attempted to kiss AV, which resulted in unreasonable discomfort and a loss of dignity. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
