Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. AV is care planned to be redirected and staff are to have eyes on AV when he/she is out of his/her room or in the dining area. On or about June 29, 2025, AV grabbed W1 and they started hitting each other. No staff were in the dining area observing the residents at the time of the altercation. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
