Findings: The Alleged Victim (AV) had money in an envelope in his/her room. On or about June 28, 2025 AV informed staff that there was money missing from his/her room. An investigation could not determine the person who took AV's money. Alleged Perpetrator #2 IAP2, Unknown) took AV's money from his/her room. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility's failure to protect AV from financial abuse is a violation of Oregon Administrative Rules.
