Findings: The Alleged Victim (AV) relies on the facility for his/her care and has a history of falls resulting in major injuries. According to an investigation, on or about June 30, 2025, AV experienced a fall, resulting in a neck fracture. The facility failed to appropriately plan care to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
