Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV has a history of resident-to-resident incidents and is known to wander around the facility and into other residents’ rooms. According to an investigation, on or about July 2, 2025, staff heard yelling coming from W1’s room and found W1 dragging AV by the hair out of W1’s room. AV experienced scalp irritation and unreasonable discomfort. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-01309 $169.00 fine assessed
