Findings: The Alleged Victim (AV) requires assistance for toileting. Staff are to assist AV to and from the toilet. AV will use his/her call light to request assistance from staff when he/she is ready to be removed from the toilet. On multiple occasions during the month of June 2025, AV's call light was not answered between 30 minutes to over one hour, causing AV unreasonable discomfort. The facility's failure to answer the call light timely is a violation of resident rights, is considered neglect of care and constitutes abuse.
