Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation AV was struck in the head and neck by Witness 1 (W1) while facility staff watched W1 approach AV and strike them in the head. Both AV and W1 have a history of behaviors and resident-to-resident altercations, but the facility failed to implement interventions to keep AV safe. This is a violation of resident rights and constitutes neglect, which is abuse.
