Findings: The Alleged Victim (AV) relies on the facility for medication administration. According to an investigation, AV had two teeth extracted on July 8, 2025, and was provided with orders for pain medication. The pain medication was not administered to AV until July 10, 2025. Between July 8, 2025, and July 10, 2025, AV experienced increased agitation and was the aggressor in a resident-to-resident altercation. The facility failed to administer medications as ordered for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
