Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) who has a known history of exit seeking. On or about July 13, 2025, an outside vendor was provided the door access code so they could go in and out of the secured facility. When the vendor was leaving, the AV followed them out of the facility. The AV was located approximately 15-30 minutes later about a block away. The failure to provide a safe environment led to the AV leaving the secured memory care unattended, creating a risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
