- Findings
- AV is dependent on the facility to ensure a safe environment, and provide services necessary for AV's health and safety. AV has a documented history of falls and balance-related issues. According to AV’s care plan, the following fall prevention interventions are in place: Staff are to ensure that AV’s environment is free of clutter; AV is to use a 4-wheel walker for all ambulation; If AV is observed ambulating without the walker, staff are to immediately cue AV to sit and retrieve the walker before AV continues ambulating. On or about July 14, 2025, AV was walking through the front entrance of the hub without wearing shoes and without the use of h/h 4-wheel walker. At the time, another resident had experienced an accident resulting in a puddle on the floor. Staff were attending to that resident and had not yet cleaned the puddle off the floor, AV slipped in the puddle, fell, and struck the back of their head on the floor. A bruise was later observed on the upper right posterior area of AV’s head. The facility did not maintain a safe environment by failing to promptly clean up the puddle, and facility staff did not ensure that AV was using the required assistive device, as outlined in AV’s care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
