Findings: The Alleged Victim (AV) relies on the facility for care. AV was care planned for shower assistance, incontinence care, other Activities of Daily Living. According to an investigation, between October 2024 and July 2025, AV did not consistently receive assistance with personal care and developed a rash in their thigh and groin area and AV's toenails were cutting into AV's skin and had a fungal infection. The facility failed to provide services, to include implementing interventions and or care planning to mitigate AV’s behaviors, resulting in a repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP26-00219 $1500.00 fine assessed
