Findings: The Alleged Victim (AV) relies on the facility to ensure a safe and secure environment. The AV is known to exit seek from the secured facility and has a history of attempting to elope. According to an investigation, on or about July 17, 2025, the AV eloped from the secured facility and was unsupervised in the unsecured portion of the facility, placing them at risk of harm. The facility failed to provide a secure environment, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
Sanction: RCFCP25-01031 $188.00 fine assessed
