Findings: On or about July 18, 2025, at approximately 12:35 AM, the Alleged Victim (AV) was found on his/her fall mat. AV is care planned for safety checks hourly. AV had last been seen at approximately 10:30 PM sitting on the bed with his/her head on the pillow and feet on the floor. AV was not harmed in the fall but was at risk for harm due to failure to follow the care plan for hourly checks. Respondent failed to ensure staff followed the care plan. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
