Findings: Alleged Victim (AV) relies on the facility for assistance with incontinence care. According to an investigation, on or about, July 18, 2025, the AV was identified with friction wounds on July 7, 2025. On July 18, 2025, the AV was identified to have debris from a brief that had broken due to oversaturation in their genital area, was lying on a soiled incontinence pad, and a new pressure wound. The AV's service plan lacked direction for staff related to the provision of incontinence care. The facility failed to properly care plan and provide appropriate services for the AV, posing a serious risk of harm and lack of personal dignity, which is a violation of resident rights, is neglect of care, and constitutes abuse.
