Findings: On or about July 19, 2025, the facility failed to ensure the safe administration of medications as ordered, resulting in the Alleged Victim (AV) not receiving their prescribed anticonvulsant medication. This medication is critical for managing AV’s seizure condition, and the missed dose placed AV at risk of moderate harm. The facility's failure to provide a safe medication system for AV is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
