Findings: The Alleged Victim (AV) has a known exit seeker. On or about July 19, 2025, AV was found unattended outside the facility at approximately 7:30 am. There are conflicting statement of how AV exited the secured facility. The facility is located next to a busy road, and AV being outside of the secured community placed AV at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
