Findings: Alleged Victim (AV) and Witness 1 (W1) are residents of the facility and rely on staff to meet all care needs. W1 has history of resident-to-resident altercations. On or about July 20, 2025, W1 and AV were involved in a resident-to-resident altercation where W1 smacked AV in the face. AV complained of mild pain to the right side of AV's face and had a slight red mark. The facility failed to provide a safe environment for AV and failed to implement person centered interventions for W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse. which is a violation of resident rights, is considered neglect of care and constitutes abuse.
