Findings: On or about July 25, 2025, the Alleged Victim (AV) sustained a fractured arm following an unwitnessed fall. Based on facility documentation and interviews, the facility failed to provide appropriate supervision and failed to follow AV's care plan, specifically regarding scheduled toileting and safety checks. The facility's failure to provide necessary care and services, resulted in unreasonable discomfort and physical harm to AV, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
