Findings: Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system to ensure the Alleged Victim's (AV) medications were administered as ordered. On or about April 26, 2025, AP2 administered AV an additional does of metabolic medication. AP2 did not check AV's MAR before administering the medication. AV started exhibiting adverse reactions, EMS was called, and AV was transported to the hospital for observation and treatment. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules
