Findings: Witness #1 (W1) and the Alleged Victim (AV) have a history of altercations, which occurred on July 3, 2025, July 28, 2025 and July 30, 2025. On or about August 3, 2025, AV was walking down the hallway and W1 was sitting in a chair. W1 got up when he/she saw AV and pushed AV into the wall. After the altercation, AV was found on the floor curled up in a ball due to anxiety and needed as needed medication. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01171 $375.00 fine assessed
