Findings: Alleged Victim (AV) had exit-seeking behaviors between August 1, 2025, to August 5, 2025. On or about August 5, 2025, at around 6:00 pm, AV eloped out of the memory care exit doors. AV was found on the sidewalk outside of the memory care exit doors. AV previously eloped from memory care on August 1, 2025. On August 5, 2025, prior to AV’s elopement, AV was taken off of alert charting for elopements despite AV displaying exit-seeking behaviors for the previous 5 days. Respondent failed to implement meaningful interventions for falls, to ensure the safety of AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
