Findings: The Alleged Victim (AV) and Witness 1 (W1) have history resident-to-resident altercations, including recent altercations with each other. Alleged Victim (AV) is known to introduce themselves to others. W1 has a history of becoming frustrated and upset. According to an investigation, on or about June 30, 2025, AV extended their arm towards AV to shake W1’s hand when W1 grabbed AV’s hand and squeezed it tightly. After W1 released AV’s hand, AV’s pinky finger was red. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
