Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations with each other. On or about August 7, 2025, while wandering the hallway, W1 approached AV and bit AV's finger, causing swelling and bleeding. The facility's failure to provide a safe environment caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
