Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on or about August 10, 2025, AV was actively attempting to exit the secure facility. The facility failed to ensure that doors were locked and functioning property, which allowed AV to elope from the facility. This is a violation of resident rights, is considered neglect of care which constitutes abuse.
