Findings: On or about August 11, 2025, it was discovered that narcotic medication belonging to the Alleged Victim (AV) was being signed out multiple times per day, when AV's medication record states to take 1/2 tablet two times per day as needed. Alleged Perpetrator #2 (AP2) was the staff member who logged out the medication according to facility records. An investigation determined that AV did not receive these medications, however, these medications were missing. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes financial abuse. The facility's failure to ensure a safe medication administration system and protect AV from financial abuse is a violation of resident rights, is considered neglect of care and constitutes abuse.
