Findings: Alleged Victim (AV) has a known history of falls. On or about August 5, 2025, AV had an unwitnessed fall while trying to get out of bed to use the bathroom when AV slipped and hit AV’s head on the bed frame before landing on AV’s bottom. As a result of the falls, AV was diagnosed with spinal fractures. The facility failed to update AV’s Service Plan to document and properly plan for AV’s fall risk, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-01041 $500.00 fine assessed
