Findings: The Alleged Victim (AV) relies on the facility to assist with managing care of their catheter. According to an investigation, the AV was admitted to the facility with a catheter in place on or about June 6, 2025. The AV's Service Plans, dated June 6, 2025, and July 5, 2025, indicated the facility care team would coordinate heath care visits and services and Home Health for catheter care. Home Heath catheter care was not initiated or provided between approximately June 6, 2025, and July 11, 2025. The AV was sent to the emergency department on approximately June 25, 2025, and July 9, 2025, due to symptoms and discomfort related to their catheter. The failure to coordinate care and ensure timely treatment is a violation of resident’s rights, is neglect of care and constitutes abuse.
