Findings: According to the documentation, the facility failed to reorder the Alleged Victim’s (AV) blood thinning medication resulting in not having the medication available. On or about August 09, 2025, the facility did not have any blood thinning medication for the AV. it was found the facility failed to follow protocols for reordering medication resulting in running out of this medication. The facility failed to reorder medication timely, resulting in the AV to miss a day of blood thinning medication, placing them at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00411 $500.00 fine assessed
