Findings: The Alleged Victim (AV) relies on the facility to provide a secure environment. According to an investigation, on or about August 18, 2025, the AV was noted to to exhibit exit-seeing behaviors on August 7, 2025. There were no care plan updates or instructions provided to staff related to the AV's exit-seeking. On August 17, 2025, the AV eloped from the facility and was found outside the by a community member. The facility failed to properly care plan for the AV's exit-seeking behaviors, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
