Findings: Alleged Victim (AV) lives and relies on the Respondent's facility staff to ensure AV's basic care, safety and supervision requirements are met. AV has history of pain in AV's knee and back, and decreased range of motion. AV's service plan reflects staff should go slow and be attentive if AV states that something hurts during cares. However, the facility failed to ensure a safe environment for AV while receiving basic care. Alleged Perpetrator #2 (AP2) received and acknowledged receipt of trainings necessary to provide care to residents. On or about August 21, 2025, AP2 forcefully brought AV's legs up beyond what was safe or comfortable for AV while providing care to AV, causing AV pain. AP2’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failure is a violation of Oregon Administrative Rules.
