Findings: On or about August 16, 2025, Alleged Perpetrator #2 (AP2) was passing medications to the Alleged Victim (AV). AP2 crushed AV's medication, which is not to be crushed, as it's a time release medication. AP2 was not trained to pass medications by the facility prior to passing medication. AV suffered unreasonable discomfort due to the medication being crushed. The facility's failure to provide a safe medication administration system and properly train staff is a violation of Oregon Administrative Rules.
