Findings: On or about August 12, 2025, Alleged Victim (AV) discovered $50.00 was missing from his/her wallet. Based on witness statements, investigator observations, and documentary evidence collected during Adult Protective Services (APS) Investigation #00422069, the APS Investigator determined that AV's money was taken by an unknown individual, and this person is responsible for theft of property which is considered financial exploitation and constitutes abuse. The facility failed to protect AV's property from theft which is a violation of Oregon Administrative Rules.
