Findings: The Alleged Victim (AV) relies on the facility for care, supervision, and safety. On or about August 26, 2025 , facility staff discovered AV was missing from the facility. According to an investigation, AV had left the facility, seemingly through an unlocked/unlatched courtyard gate, and walked approximately 4 blocks down a busy road to a local gas station/ convenience store. It is estimated AV was outside of the locked facility for approximately 15 minutes. The facility failed to provide appropriate services, regarding care planning and resident safety, which placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
