Findings: Alleged Victim (AV) and Witness 1 (W1) live and at Respondent’s facility and rely on facility staff to provide care and supervision. W1 has a history of resident-to-resident altercations with other residents at Respondent's facility, including incidents that occurred in June and August 2025. On or about August 26, 2025, W1 wandered the halls and approached AV, a brief verbal exchange occurred, W1 swung, attempted to kick AV, and grab AV's throat. Respondent failed to develop and implement suitable interventions which constitutes abuse by neglect.
