Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV has known visual deficits and has a history of unwitnessed falls that occurred in AV’s room. According to an investigation, between August 19, 2025, and August 27, 2025, AV complained of pain and AV’s hip area appeared swollen. AV was sent to the hospital where AV was diagnosed with a fracture and a blood clot in the hip area. The facility failed to appropriately plan care and/or implement interventions to mitigate the risk of injury, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP25-01474 $1500.00 fine assessed
