Findings: The Alleged Victim (AV) relies on the facility to manage his/her medications. According to an investigation, on or about August 28, 2025, AV did not receive their medication as ordered for approximately three days, resulting in risk of harm. The facility failed to provide a safe medication administration system. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
