Findings: The Alleged Victim (AV) is a fall risk and has a history of falls. According to an investigation, the AV was identified with fractured ribs on or about July 24, 2025, caused by a fall on or about July 19, 2025. There were no interventions developed or implemented following this injury fall. The AV was found on the floor on or about September 4, 2025. The AV was sent to the emergency department and diagnoses with a brain bleed, fracture to the left orbital floor, facial laceration, and closed fracture of left maxillary sinus. The facility failed to appropriately service plan and implement interventions to mitigate the risk of injury due to AV's falls, which is a violation of resident rights, is neglect of care, constitutes abuse.
