- Findings
- Alleged Victim (AV) relies on the facility for his/her care. On or about September 5, 2025, the Alleged Perpetrator 2's (AP2) approached AV in the common area dining room, rubbed AV's arm and kissed AV's forehead, then walked away. According to an investigation, AP2 acknowledged that kissing AV was unprofessional, but AP2 was trying to convey they cared for AV. AP2 actions are considered sexual abuse. The facility failed to provide a safe environment for the AV which is a violation of Oregon Administrative Rules.
