- Findings
- According to the documentation, Alleged Perpetrator 2 (AP2), Alleged Perpetrator 3 (AP3), and Alleged Perpetrator 4 (AP4) failed to follow facility policy to ensure no residents left the secured building behind them and allowed the Alleged Victim (AV) out of the secured memory care. The AV is care planned to have exit seeking behaviors. On or about September 06, 2025, AP2, AP3 and AP4 were exiting the building after their shift ended and did not follow facility procedures to check for residents potentially following them out. During this time, the AV was able to exit the facility behind the three staff members. one of the AP’s noticed the AV walking along the sidewalk when they were by their car. The AP was able to redirect the AV back into the facility. The failure to follow facility procedures to ensure residents did not leave the secured memory care unattended is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
