Findings: The Alleged Victim (AV) is a known fall risk and has suffered 9 injury falls in 4 months, resulting in skin tears, bumps to the head and bruising. On or about September 7, 2025, AV was found on the floor by staff at approximately 3 AM, with scratches down his/her back. AV's interim service plans do not have specific interventions to prevent AV from suffering falls. The facility's failure to properly care plan to prevent falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
