Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about September 2, 2025, AV started showing signs/symptoms of a UTI and experienced a fall on or about September 7, 2025. AV experienced pain and was not taken for a medical evaluation until approximately September 8, 2025, where AV was diagnosed with a UTI. The facility failed to assure timely medical treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
